Please complete this form and submit it to the church office as soon as possible. Your facility use request will be reviewed by the staff of Oasis Community Church.

IN ORDER TO HOLD YOUR DATE ON THE CHURCH CALENDAR, THIS FORM MUST BE COMPLETED AND SUBMITTED TO THE CHURCH. APPROVAL OF THE EVENT REQUESTED IS SECURED WHEN THE LEAD AND/OR ASSOCIATE PASTOR REVIEWS AND VERIFTYS THIS FORM.

I/ We have received and read the Oasis Community Church Facility Use Policy, as well as any extra statements requested of Oasis Community Church and the Christian & Missionary Alliance. With this knowledge, I/we request the use of the OCC Facility. I/We understand that a facility use fee may be required.
